What to Expect Before Your Appointment

welcome-new-patient.jpgWhen you are scheduled to see an OSF Health Cardiovascular Institute provider as a new patient, you will receive the following in the mail to fill out prior to your appointment.

  • Appointment letter
  • Patient history form
  • Personal Representative Designation Form (PRD)

If your appointment is scheduled within the next three days, you will not receive anything in the mail and will be asked to come to your appointment early to fill out these forms. The PRD is a form where you, as the patient, designate any relatives or friends that we can release health information to in the event that someone should need to contact us on your behalf.

You will receive a telephone call a few days before your visit to complete the pre-registration process. At that time, you will be asked if you would like another phone call within 24 to 48 hours of your appointment to remind you of your appointment. If your appointment is scheduled within the next couple of days, you will not receive another call. All registration will be completed when you come in for your appointment.

What to Bring to Your Appointment

Please bring the following to your appointment:

  • All your medications in the original containers, including vitamins and herbal supplements you may take
  • Your insurance and/or Medicare card

What to Expect During Your Appointment

When you arrive, please check in with the receptionist. We know your time is valuable and make every effort to keep appointments as close to your scheduled time as possible. However, in health care, emergencies sometimes arise. On the rare occasion that your appointment is delayed, we will keep you informed about the delay.

Your first visit will include measurement of weight, blood pressure and pulse, and a review of medications and allergies by a medical office assistant. Then, you will see the provider, who will discuss with your medical history and current condition with you. The provider will also do a physical exam.

在考试后,供应商将讨论结果and recommendations with you. A nurse will provide further education as needed and review the provider's recommendations.

If testing is required, you will meet with the scheduler to make an appointment. The provider will communicate findings and recommendations to your primary care provide as well as the referring provider - if the referring provider is not your primary care provider.

Should you have any questions before your appointment, please call the office where your appointment is scheduled. If you have urgent questions or you experience a change in symptoms before visiting OSF HealthCare Cardiovascular Institute, please contact your family provider.

How to Cancel Your Appointment

If you need to cancel your appointment, we request that you call 24 hours in advance, whenever possible. If you cannot call at least 24 hours ahead, please call as soon as you know you need to cancel. Another patient may be able to use your appointment time.
