Community Partnerships
To determine the needs of the community, leaders facilitated town hall meetings, utilized direct mailing, distributed surveys and developed a committee dedicated to creating a healthy community. Ideas have also been gleaned from other successful, innovative rural health care delivery models. This has led to the redesign of OSF Center for Health – Streator, as it transforms into a hub that focuses more on health and wellness rather than being a facility that cares only for the sick.
A Hub for Healthy Living
The organization has enlisted a variety of community-based agencies to co-locate within Center for Health, making the facility a convenient space for health and wellness needs. Current partnerships include:
Cancer Resource Center & Wig Boutique
Cancer Resource Center and Wig Boutique sponsored by the Streator Park Presbyterian Church is located on the third floor of the OSF HealthCare Center for Health - Streator at 111 Spring Street. Staffed and operated by volunteers, many of whom are cancer survivors themselves, the Patient Resource Center is open on Mondays and Wednesdays from 10 a.m. to 2 p.m., call (815) 673-5620. Services including a wig boutique and educational materials are available to cancer patients from anywhere within the county.
The wig boutique has over 200 wigs in stock so a patient can walk out, same day with a wig. Two local hair stylists also provide free styling of the wigs, if needed, by appointment. Among other items that are available to patients and families are hats, scarves, breast pillows, and a wide array of educational materials, on all types of cancers. This includes information on nutrition as well as how to talk to your doctor and family about the journey a cancer patient must take.
LaSalle County Health Department
The LaSalle County Health Department has a satellite clinic for two of their programs – Women, Infants, and Children (WIC) and Family Case Management (FCM) on the third floor of the OSF HealthCare Center for Health - Streator at 111 Spring Street. Both the FCM and WIC programs are funded by the Illinois Department of Human Services. The FCM program focuses on promoting healthy families through education, screenings, assessments, referrals, and other services based on client need. The WIC program serves income-eligible pregnant, post-partum, breastfeeding women, infants, and children up to age five years. The WIC program provides health screenings, nutrition education and counseling, and supplemental foods including formula for infants.
North Central Behavioral Health Systems
North Central Behavioral Health Systems opened an office co-located on the third floor of OSF HealthCare Center for Health - Streator. North Central Behavioral Health Systems is a nonprofit corporation providing a wide variety of community education, prevention, mental health, and substance treatment services through ten service location facilities in Central and North Central Illinois as well as intensive home and community-based and related services. The organization serves over five thousand patients per year from more than one hundred communities in La Salle, Bureau, Putnam, Marshall, Stark, Fulton and McDonough Counties
Guardian Angel Outreach
This is a parish ministry established by St. Michael the Archangel Catholic Church in collaboration with Catholic Charities. Our outreach assists at-risk mothers and their infants from conception through the first three years of the baby’s life. Our doors are open to assist all women facing new-life issues from the city of Streator, IL and surrounding communities. Individuals will find a safe and caring environment with dedicated volunteers and staff.
Tri-County Opportunities Council - Building Opportunities to Transform Communities
Our Mission: Tri-County Opportunities Council will investigate the impact of poverty throughout our nine county service area and will work, in partnership with individuals, families and communities to provide opportunities that support movement toward stability and self-sufficiency. Located on the third floor of the OSF Center for Health - Streator offering Community Services Block Grants (CSBG) and Low-Income Home Energy Assistance Program (LIHEAP) is designed to help income eligible households pay for the rising cost of winter energy services.
