Fill in your name and date of birth and clickSubmit.

You will receive a confirmation email to the address you used to sign up.

Fill in your username and date of birth. ClickNext. A one-time code will be emailed or texted to you.

Enter the code onto the form and clickNext. You will be prompted to create a new password.

ClickNextto complete the process.
