OSF HealthCare is pleased to add new primary and psychiatric care providers to its staff to better serve the community of Princeton and surrounding areas.
The addition of Dr. Travis Swink, DO, Jennifer Gutshall, PA, and Kellie Kozlowski, APRN, on July 24 will ensure increased access for patients in need of primary care and psychiatry services at the OSF Medical Group office at 530 Park Ave. E.
Dr. Swink, Gutshall and Kozlowski offer a number of services including chronic disease management, preventive care and mental health care support and treatment.
New or existing patient appointments with Dr. Swink, Gutshall or Kozlowski can be made by calling (815) 876-3350. The office will be open five days a week. Clinic hours are Monday, 7:30 a.m.-4 p.m., Tuesday and Thursday, 7 a.m.-6 p.m., and Wednesday and Friday, 7:30 a.m.-6 p.m.
