One example of philanthropic support for patients and the community at large, The Community Resource Center (CRC) provides referral services to clients and helps connect them to an array of social services available in the community. Comprised of 2-3 full-time health navigators, the center helps patients find the most appropriate source of care for their needs. This in turn benefits the hospital by reducing unnecessary utilization of the emergency department. When you support the Community Resource Center, you help patients:

• Find a primary care provider, secure health insurance and locate dental services
• Afford prescribed medications, obtain housing and other financial resources
• Manage behavioral health, locate substance abuse treatment and find employment
• Navigate local senior services and secure transportation
